TermDefinition
SOAP Subjective, Objective, Assessment, and Plan
Subjective First step. The data from the patient such as, duration of problem, quality of the problem, and relieving or exacerbating factors of problem
Objective Second step. Collecting data such as physical examination, imaging or lab findings.
Assessment Third step. Formulation of a logical analysis. This could be diagnosing or identifying a problem. Also could include a list of possibilities for the diagnosis (differential diagnosis).
Plan Fourth step. This is the course of action that could include treatments such as medication, and or procedures.
Febrile To have a fever.
Afebrile Does not have a fever.
Malaise Does not feel well.
Occult Hidden.
Sequela A problem resulting from a disease or injury.
