TermDefinition
Doctor  (Hospital based or GP) diagnoses and treats physical and mental health.
Dentist  Assesses oral health and provides dental treatment. 
Pharmacist  Dispenses medication and advises on the individuals on non emergency health issues.
Nurse  Collaborates with teams to plan patient care, monitors and records the individual’s health status, administers medication and support holistic care needs.
Specialist Community Public Health Nurse Supports and promotes health and development of children and families.
Practitioner A person who works in health and social care
Allied Health Professionals Practitioners who are not doctors, nurses or dentists. Examples include paramedics, physiotherapists, occupational therapists and dieticians.
Physiotherapist assesses and supports the individual affected by injury, illness or disability through tailored exercise programmes, manual therapy and advice
Occupational therapist assesses and supports the individual’s physical, psychological, social and environmental needs and provides adaptations
Paramedic responds to emergency calls in the community, assesses the individual and provides life-saving medical intervention
Dietitian assesses and provides nutritional advice to promote a balanced diet.
Social worker ​ Works in partnership to assess and support individuals in need to safeguard and protect from harm​
Social care prescriber​ Signposts individuals to community support for wellbeing​
Outreach worker ​ Provides emotional and practical support to individuals within the community to help them take part in all aspects of everyday life​
Family support worker​ Establishes relationships with individuals and families in need to provide tailored support.​
Care assistant  Provide support and routine care e.g. washing, dressing, making beds, communicating and cleaning
Activity coordinator Plan and deliver activities and entertainment for people in care to participate in - either in care homes or trips away.
Speech and Language therapist Help manage disorders of speech, language, communication and swallowing in children and adults.
Person-centred practice putting individuals at the centre of their care.

Care consistent tailored care throughout life.
Compassion how care is underpinned by emphatic, respectful and dignified relationships.
Competence delivery of evidence-based care and treatment.
Communication key to caring relationships and facilitating team working.
Courage to raise concerns and be open to innovative ways of working.
Commitment dedicated to improving care and experience of the individual and embrace future challenges.
Care values that underpin practice include... ...Duty of care, safeguarding, dignity, respect, rights, confidentiality and independence
Duty of care - Maintains legal requirements to protect the individual and act in their best interests. e,g, only doing work they are trained to complete.
Safeguarding - Ensures safety of the individual and protects from harm and abuse. e.g. reporting any incidents
Dignity - Promotes the individuals self-respect. not intentionally or unintentionally embarrassing them. e.g. closing doors when giving personal care
Respect - Acknowledges diversity through recognising and responding to the individual’s needs and preferences. e.g. knocking on door, don't sit on their beds and communicating with users.
Rights - Promotes entitlements set out in law. e.g. never discriminate and the users have a right to refuse care.
Confidentiality - Maintains privacy and security of personal information. e.g. not sharing information on social media. Information is stored under passwords online
Independence - Enables the individual to make own decisions. e.g. bath/shower preferences. what to eat and what to wear etc.
Continuing Professional Development (CPD) Learning and developing new skills and knowledge throughout your career.​ All employees in health and social care have the right to learn and will have CPD.
