S.O.A.P Method Subjective: what the patient says Objective: what the test reveal Assessment: the analysis of the subjective & objective information; preformed by a healthcare provider Plan: the course of action for the patient
Subjective Term: Acute vs. Chronic Acute: severe & sudden in onset Chronic: something that continues over time
Subjective Term: Febrile vs. Afebrile Febrile: relating to fever Afebrile: having no fever
Objective Term: Things that are felt- Palpation Method of feeling with the fingers or hands during a physical examination
Objective Term: Things that are seen- Alert & Oriented Alert- the patient is aware of the examiner & can respond to the environment around them independently Oriented- person's level of awareness of self, place, time, and situation
Assessment Term: Benign vs. Malignant Benign- a condition, tumor, or growth that is not cancerous Malignant- the presence of cancerous cells that have the ability to spread to the other sites in/on the body
Assessment Term: Diagnosis The process of determining the nature of a disease or disorder
Assessment Term: Lesion A change in the structure of a bodily organ or part due to injury or disease
Plan Term: Observation The assessment of patient signs during a physical examination
Plan Term: Reassurance The removal of fears & concerns about illness
