What does the S in SOAP stand for? Subjective (what the patient says)
What does the O in SOAP stand for? Objective (what the tests reveal)
What does the A in SOAP stand for? Assessment (the analysis of the subjective and objective info)
What does the P in SOAP stand for? Plan (course of action for the patient)
What is "Hx"? History
What is "PMHx"? Past Medical History
What is "FHx"? Family History
What does PCP stand for? Primary Care Provider
What does CC stand for? Chief Complaint
What does NKDA stand for? No Known Drug Allergies
