Pain management
Unit 4 section 5
| Question | Answer |
|---|---|
| What is a noxious stimulant? | Something that damages or destroys tissues |
| What do nociceptors do? | Transmit pain message to brain via the spinal cord |
| What is peripheral sensitation? | Reactive nociceptors excite dormant ones to respond |
| What is central sensitization? | Heightened response found in the spinal cord. |
| What are the 4 basic processes involved in nociception? | Transduction, transportation, modulation & perception |
| What is neuropathic pain? | Pain caused by damage to the nervous system |
| What is inflammatory pain? | Clinical pain initiated by tissue damage & inflammation |
| What are some negative consequences of pain? | Tachycardia, hypertension, increase of oxygen consumption |
| What is released when pain is percieved? | Catecholamines, cortisol & glucose |
| What are some physiological effects of pain? | Increased temperature, panting, dilated pupils, shaking/shivering |
| What type of analgesia is described as the cornerstone of effective postoperative pain treatment? | Opioids |
| What are the 3 receptors opioids react on? | Mu, delta & kappa |
| What is the most potent opioid? | Full mu opioid agonst |
| What is an example of a full mu opioid agonist? | Methadone, morphine, fentanyl, pethidine |
| How long does onset happen for partial mu opioid agonists? | 20-30 mins |
| What schedule is buprenorphine? | schedule 3 |
| What is an example of a full mu agonist? | Naloxone |
| What is an example of a NMDA antagonist? | Ketamine |
| When is use of ketamine indicated? | When neuropathic or chronic pain is present |
| What type of analgesia produces a synergistic effect when given with opioids? | Alpha-2 agonist; medetomidine |
| What are some side effects of medetomedine? | Bradycardia, hypotension, vasoconstriction, hypoventilation, reduced tear production & increased diuresis |
| What does NSAID stand for? | Non Steroidal Anti Inflammatory Drugs |
| What enzyme do NSAIDs specifically block? | Cyclooxygenase (COX) pathway |
| What are some contraindications of NSAID use? | Dehydration, hypovolaemic, low BP, kidney disease, GI upset, concurrent steroid use. |
| What drugs should never be used in cats? | Paracetamol |
| How does paracetamol work? | Blocks the chemical receptors in the brain to prevent perception of pain. |
