Question | Answer |
---|---|
Shoulder - Apley's Scratch Upper | Restriction in GH ROM by abduct and externally rotate shoulder. Touch top of opposite scap. Pos=inability means weakness, tightness or capsular restriction |
Shoulder - Apley's Scratch Lower | Restriction in GH ROM by adduct & internally rotate shoulder. Touch bottom of oppsite scap. Pos=inability means weakness, tightness or capsular restriction |
Shoulder - Painful Arc Syndrome | Altered mechanics GH Jt by moving thru arc of abduction. Raise arm. Pos=pain. 60-120 degrees=pinch GT and coracoacromial arch. 160-180 degrees=AC jt dysfunction. |
Shoulder - Speed's Test | Bicipital tendonitis by loading mm. Supinate & extend arm - while therapist pushes down (isometrically) POS=Px in bicipital groove or distal attachment. |
Shoulder - Yergason's Test | Bicipital tendinitis & stability of tendon by loading mm and trying to twist tendon out of groove. Elbow 90 - push down on hand + ext rot. POS=Px or tendon pops out = instability of transverse humeral ligament. |
Shoulder - Drop Arm Test | Tear in supraspinatus mm/tendon by controlled contraction of mm. From fully abducted, slowly lower arm to side. POS=exquisite pain or weakness |
Shoulder - Supraspinatus Test (Empty Can Test) | Tear of supraspinatus mm/tendon by loading mm. 90 degree horizontal-press down isometrically. Then adduct 30 degrees and reapply pressure. POS=pain and/or weakness indicates lesion of mm/tendon |
TOS - Adson's Maneuver (Scalene Anticus Syndrome) | Nerve impinged between ant and middle scalenes by tightening up scalenes. Stand behind, monitor radial pulse, passively extend and ext rot arm, turn head same side POS= weakened pulse, paresthesia |
TOS - Eden's Maneuver (Costoclavicular Syndrome Test) | Nerve impinged between clavicle & 1st rib by elevate and retract clavicle. Monitor Radial pulse, raise shoulder, swing it back and lower it + apply downward force. POS=same as Adsons |
TOS - Wright's Maneuver (Hyperabduction Test, also Pec Minor Syndrome Test) | Nerve impinged between pec minor & coracoid proc by putting tension on tendon of pec minor. Monitor radial pulse, passively hyperabduct arm. POS = same as Adson's |
Shoulder - Anterior Apprehension Test (Crank's Test) | Suspected ant shoulder sublux or dislocation by mimicking dislocation position. Passively bring arm 90 abduction and forceful ext rot. POS=facial expression of apprehension + mm guarding |
Elbow - Valgus Stress Test | Stability of med collateral lig by stressing MCL (stretch). Hold lat aspect of elbow and med aspect of wrist - push elbow in. POS=pain or hypermobility |
Elbow - Varus Stress Test | Stability of lat collateral lig by stressing LCL (stretch). Hold med aspect of elbow and lat aspect of wrist - push elbow out. POS=pain or hypermobility |
Elbow - Tests for Medial Epicondylitis (Golfer's Elbow) | Tendonitis of forearm flexors by applying 3 stresses. Palpate tendon, passively stretch (extension) and iso resist flexion (fist). POS=pain on palpation, stretch and resisted action = tendonitis |
Elbow - Tests for Lateral Epicondylitis (Tennis Elbow) | Tendonitis of forearm extensors by apply 3 stresses. Palpate tendon, passively stretch (flexion) and iso resist extension (fist in ext). POS=pain on palpation, stretch and resisted action = tendonitis |
Wrist/Hand - Allen's Test | integrity radial&ulnar arteries by compressing one side. Pump hand 10 times, therapist wraps fingers around wrist and releases one side. Repeat with opp side. Compare. POS=one side takes much longer or does not supply blood to hand = insufficiency |
Wrist/Hand - Finkelstein's Test | DeQuervain's Tenosynovitis by passive stretch of APL & EPB over radial styloid process. Tuck thumb into fist & ulnar deviate wrist. POS=pain over radial styloid proc = tendonitis |
Wrist/Hand - Murphy's Sign | Dislocation of lunate by checking alignment of metcarpals. Examine height of metacarpal heads in fist. POS=3rd metacarpal is even or lower=lunate dislocated |
Wrist/Hand - Phalen's Test | CTS by compressing carpal tunnel to reproduce symptoms. By max flex wrists - backs of hand together and flexing wrists. POS=paresthesia palmar surface (thumb side half of hand)=CTS |
Wrist/Hand - Reverse Phalen's Test | CTS by compressing carpal tunnel to reproduce. Palms together and wrists in extension (praying). POS=paresthesia palmar surface (thumb side hand)=CTS |
Wrist/Hand - Tinel's Sign | Irritation of median nerve (CTS) by striking the nerve. Tap palmar surface at carpal tunnel. POS=electric pain up arm or down into hand, paresthesia = CTS |
TMJ - Jaw Reflex | Integrity Trigeminal nerve CN V by eliciting reflex contraction of masseter & temporalis. Jaw open, relaxed - thumb on chin, press down slightly - hit thumb w reflex hammer. POS=abnormal (hypo or hyper) reflex |
TMJ - Chvostek Test | Integrity Facial nerve CN VIII by tapping over area of nerve. Tap over masseter mm. POS=twitching of mm on side of face. |
TMJ - Three Finger or Three Knuckle Test | Detect resistance in opening of jaw by testing extent of opening range. Attempt to insert three stacked fingers (theirs) into mouth. POS=inability to open wide enough=restriction in active ROM |
