Four T's

Basic Assessment 4T's

QuestionAnswer
Temperature Tissue may be hot, indicating inflammation, or cool, indicating ischemia.
Texture Swelling or edema is present
Tenderness Pain can be indicated if the client winces or pulls away with tissue compression.
Tone Tissue may be hypertonic = increase in tone relative to nearby muscles - or hypotonic = decrease in tone..
