| Polypharmacy | use of multiple medications, the use of potentially inappropriate or unnecessary medications, or the use of a medication that does not match a diagnosis |
| Idiosyncratic reaction | an overreaction or underreaction to a medication or a different reaction than normal. |
| Medication reconciliation | process where nurses, pharmacists, and other health care providers compare the medication that a patient is currently taking with what the patient should be taking and any newly ordered medications. |
| Intradermal injection | a shallow or superficial injection of a substance into the dermis |
| Parenteral | involves injecting medications into body tissues |
| Subcutaneous injection | injection into tissues just below the dermis of the skin |
| Buccal | medication administered by placing solid medication in the mouth against the mucous membranes of the cheek until it dissolves |
| Sublingual | route of medication administration in which medication is placed under the tongue until it dissolves |
| Transdermal Disk | medication delivery device in which the medication is saturated on a wafer-like disk, which is affixed to the patient’s skin. Ensures the patient receives continuous level of medication |
| Prone | Position of the patient lying face down |
| trochanter roll | rolled towel support placed against the hips and upper leg to prevent external rotation of the legs |
| STAT order | signifies that a single dose of a medication is given immediately and only once. |
| Now order | used when a patient needs a medication quickly but not right away, the nurse has up to 90 minutes to administer the medication. |

| Paraplegia | Paralysis of the lower portion of the body and of both legs from a severe cord injury in the lumbar region |
| Cellulitis | an acute, spreading inflammation of the deep subcutaneous tissues |
| quadriplegia | paralysis of the arms and legs |
| Dysphasia | a speech impairment in which there is a lack of coordination and an ability to arrange words in their proper order. Typically the result from a brain lesion. |
| Aphasia | Inability to communicate through speech, writing, or signs |
| charting by exception (CBE) | Charting methodology in which data are entered only when there is an exception from that which is normal or expected; reduces the time spent documenting in charting. |
| SOAP Note | Progress note that focuses on a single patient problem and includes subjective and objective data, analysis, and planning |
| Sitz bath | Bath in which only the hips or buttocks are immersed in fluid |
| Alopecia | Loss of hair due to effects of chemotherapy medications, hormonal changes, or improper hair care practice |
| Partial bed bath | bed bath that consists of bathing only body parts that would cause discomfort if left unbathed such as the hands, face, axillae, and perineal area. |
| Trendelenburg's | position where the entire bedframe tilted with the head of the bed down. Used for postural drainage and facilitating venous return. |
