22-Increased Procedual Services the work required to provided a service is substantially greater than typically required, doucumention to support Note: not used on E/M services
23--Unusaual Anesthesia a procedure which usually requirs either no anesthesia or local anesthesia but bc unusual circustances must be done uner gener anesthesia
24--Unrelated E/M service by same physician or other QHCP during post-op period to indicate that an E/M service was performed during a post-op period for reason(s) unreleated to original procedure
25--Significant, Separately Identifjiable E/M service by same physician or other QHCP on same day of procedure or other sevice patients condition required a significant, separtely identifiable E/M service above and beyond the usual pre-op or post-op care. may be prompted by sysmpton or condition and a different diagnosis are not required for reporting the E/M on same day
26--Professional Component when the professional component is reported separtely by physician or other QHCP
32--Mandated Services services related to mandated consultation and/or related services (third party payer, govermental, attorney)
33-Preventive Services primary purpose of the service identified is a preventive serivce identifed in preventive service mandates
47--Anesthesia by Surgeon regional or general anesthesia provided by the surgeon Note: not used as a modifier for the anesthesia procedures
50--Bilateral Procedure bilateral procedures that are performed at the same session, unless other wise identified int he listings
51-Multiple Procedures multiple procedures performed at the same session by the same idividual. lassign modifier to additional procedure listed after primary procedure
52--Reduced Services under certain circumstances a serviceprocedure is partially reduced or eliminated at the discretion of the physician or other QHCP
53-Discontinued Procedure
