Routine Venipuncture

24 steps

QuestionAnswer
Step 1 Verify the requisition for the tests.
Step 2 Identify the patient: check the patient's ID number and name & dob.
Step 3 Identify yourself to the patient , explain the procedure, and secure his/ or consent.
Step 4 Palpate the veins in the antecubital fossa, using your index finger and a tourniquet.
Step 5 Gather the necessary equipment.
Step 6 Wash hands; put on gloves.
Step 7 Tie the tourniquet. ( should be tied 4 inches above the site) - Ask the patient to make a fist to help engorge the vein.
Step 8 Palpate the vein, cleanse area using a circular motion starting at the inside of the venipuncture.
Step 9 .Assemble the needle while the alcohol is drying. - Than uncap the needle
Step 10 Hold the patient's arm, and taunt the patient's skin.
Step 11 With the bevel facing upward, insert the needle at an angle of 15-30 degrees ( IN THE SAME DIRECTION OF THE VEIN!)
Step 12 Once the needle is inside the vein, push the collection tube into the holder to puncture the tube stopper with the back end of the needle.
Step 13 Release the tourniquet once blood flow has begun. ( Never leave a tourniquet on more than 1 minute)
Step 14 Fill the needed tubes, follow the ORDER OF DRAW.
Step 15 Pull out the collection tube from the holder.
Step 16 Place folded gauze over the venipuncture site and withdraw the needle
Step 17 Discard the needle into the biohazard's sharp container.
Step 18 Label each collected specimen; patient's name, ID number, time & date of collection and your initials.
Step 19 Place labeled tubes inside the biohazards transport bag.
Step 20 Before leaving check venipuncture site:
Step 21 At any point when bleeding stops, apply band aid. (Advise patients to remove bandages within 1 hour)
Step 22 Clean up everything and dispose of waste properly.
Step 23 Leave the patient's call light within his/her reach.
Step 24 Remove gloves, wash your hands, say goodbye to the patient and tell him/ her that the physician will deliver results.
