SOAP Subjective, objective, assessment, plan
Subjective The patients initial family history, and description of problem(s)
Objective Physical exams, labs, and imaging
Assessment Diagnosis, identification of a problem, or list of possible diagnosis.
Plan Course of action. Treatment with medication or procedure. Could also consist of further data collecting for a more accurate diagnosis.
Proximal Closer to the center of the body
Distal Further away from the center of the body
Lateral To the side
Medial Towards the front of the body
Cranial Towards the top
