Abbreviations commonly used to describe symptoms or exam findings

| HEENT | Head, eyes, ears, nose, throat |
| PERRLA | Pupils are equal, round, and reactive to light and accommodation |
| NAD | No acute distress *Patient does not display any intense symptoms |
| CV | Cardiovascular |
| RRR | Regular rate and rhythm *A normal sounding heart |
| CTA | Clear to auscultation *Normal sounding lungs |
| WDWN | Well developed, well nourished *Patient shows appropriate growth and does not appear malnourished |
| A&O | Alert and oriented *patient can answer questions and is aware of what is going on |
| WNL | Within normal limits |
| NOS | Not otherwise specified * a catch-all for diagnoses that don't fit any specific cause |
