Abbreviations commonly used to describe symptoms or exam findings

HEENT Head, eyes, ears, nose, throat
PERRLA Pupils are equal, round, and reactive to light and accommodation
NAD No acute distress *Patient does not display any intense symptoms
CV Cardiovascular
RRR Regular rate and rhythm *A normal sounding heart
CTA Clear to auscultation *Normal sounding lungs
WDWN Well developed, well nourished *Patient shows appropriate growth and does not appear malnourished
A&O Alert and oriented *patient can answer questions and is aware of what is going on
WNL Within normal limits
NOS Not otherwise specified * a catch-all for diagnoses that don't fit any specific cause
