Objective Data collected to assist in understanding the nature of the problem.
Assessment Cause of the problem.
Plan Treatment with medicine or a procedure.

H & P History and Physical.
Planter Sole of the foot.
Transverse Divides the body from top to bottom.
Progressive Worsening of a symptom(s).
Malaise Not feeling well.
