Sertraline -Serotonin syndrome when mixed with Fentanyl. -agitation, tachycardia, HTN -hyper-reflexia, clonus, altered mental status -other meds that cause serotonin synd: tramadol, TCAs, opioids, dextromethorphan, antihistamines, herbals.

Clonidine -Alpha 2 Adrenergic Agonist -Most common accidental ingestion leading to hospitalization -preventive and abortive Tx for Dysautonomia in patients with Paroxysmal Sympathetic Hyperactivity
Baclofen pump -GABA agonist - -continuous delivery directly to CNS -lower dose needed -cause pressure sores, infxn, CSF leak, loss of fnxn -withdrawal and overdose -Ativan is Tx for withdrawal -
Hydrocodone -Lortab
Naloxone -only for severe events
Prochlorperazine -Migraine in ED
Olanzapine (second gen antipsychotic) Agitation
Sympathomimetic toxicity -agitation, hallucinations, tremors,seizures, hyperthermia, hypertension, mydriasis,nausea/vomit, diarrhea -
Anticholinergic ā™„ļøšŸ˜” (Benadryl, antihistamine) -anhydrotic dry skin, hyperthermia (hot as a hare) -flushed (red as a beet) -confusion, hallucinations (mad as a hatter) -pupils dilated (blind as a bat) -antidote is physostigmine
Sympathomimetic -Methylphenidate -sweat -seizures, tremors, tachycardia -hallucination, hyperthermia -agitation -diarrhea, vomit
Cholinergics Organophosphates
Oxcarbazepine -causes Hyponatremia, SIADH
-can cause acute Dystonic rxn (Dopamine blocking agent)
Diphenhydramine -Tx for acute Dystonic rxn
Sodium Benzoate -Tx of hyperammonemia -Nitrogen scavenger for excretion
-Ativan is Tx for withdrawal
Dantrolene -interfere with Ca release from sarcoplasmic reticulum -liver fnxn test
Botox A -good for spasticity -inhibits acetylcholine
Acyclovir -Neutropenia
Octreotide šŸ›‘ Stops acromegaly Stops GI bleed Stops tumor
