Terminology for Collecting Patient Data

| afebrile | without a fever |
| apnea | not breathing |
| arrhythmia | heartbeat is irregular, out of rhythm |
| bradycardia | heart rate less than 60 |
| bradypnea | respiratory rat less than 16 |
| Cheyne-Stokes | erratic breathing with periods of fast, shallow breathing followed by slow, heavier breathing and moments of apnea - often seen in dying patients |
| cyanosis | blue discoloration of skin |
| diastolic | bottom number; when heart is resting |
| dyspnea | difficulty or painful breathing |
| febrile | fever |
| hyperopia | far-sighted; can see at a distance but may need reading glasses for upclose |
| hypertension | blood pressure over 140/90 |
| hyperthermia | body temperature greatly above normal |
| hypotension | blood pressure less than 90/60 |
| hypothermia | low body temperature; below 95 degrees F |
| myopia | near-sighted; can see up close but items in the distance may be blurry |
| OD | right eye |
| OS | left eye |
| OU | both eyes |
| orthopnea | difficulty breathing when lying flat; relieved by sitting or standing |
| rales | abnormal rattling sound found on lung auscultation |
| systole | top number; when heart is pumping |
| tachycardia | heart rate greater than 100 |
| tachypnea | respiratory rate greater than 24 |
| wheezing | musical, whistling sound found on lung auscultation |
| diagnostic careers | medical professionals who create a picture of the health status of a patient at a single point in time |
| therapeutic careers | healthcare professionals who change the health status of a patient over a period of time |
| subjective assessment data | information from the client's point of view (“symptoms”), including feelings, perceptions, and concerns obtained through interviews. |
| objective assessment data | observable and measurable data (“signs”) obtained through observation, physical examination, and laboratory and diagnostic testing. |
| edema | swelling |
| palpation | a method of feeling with the fingers or hands during a physical examination |
| percussion | a method of tapping body parts with fingers, hands, or small instruments as part of a physical examination |
| peristalsis | rippling motion of muscles in the intestine or other tubular organs characterized by the alternate contraction and relaxation of the muscles that propel the contents onward |
| auscultation | listening to the sounds of the body during a physical examination |
