SOAP METHOD Subjective, Objective, Assessment, and Plan. Medical notes share a consistent pattern in their organization and layout.
Differential Diagnosis dih-fer-EN-shal DAI-ag-NOH-sis A list of possible causes of the patient's problem or complaint.
Etiology ee-tee-AW-loh-jee The cause
Pathogen PATH-oh-jin The organism that causes the problem
Prophylaxis PROH-fuh-LAK-sis Preventive treatment
Proximal PRAWK-sih-mal Closer in the center - proximal and approximate come from the same word and mean close
Distal DIH-stal Farther away from the center - distal and distant come from the same word and mean far
Prone PROHN Lying down on belly
ipsilateral IP-sih-LA-ter-al Same side
Sagittal SA-jih-tal Divides the body in slices right to left - Sagittal is Latin for arrow; think of this as dividing the body in half, as if someone shot and arrow through it
