2 Reports Date and time of specimen collection
3 Reports Description, source of specimen, and labelled precautions
4 Reports Date and hour when the procedures were completed
5 Reports Authorization, electronically or manually, by laboratory personnel performing the procedure
6 Reports Consistency in format with copies for the laboratory and the patient's medical record
7 Reports Clear instructions about interpretation or special comments
8 Reports Logical location in patient's medical record for reference laboratory reports
9 Reports Sequential order of multiple results on single specimens
10 Reports Listing of reference ranges or normal and abnormal/critical values
11 Reports Availability of laboratory charges and procedural codes for billing, coding, and other tasks
