Identity Procedure

The Basics of Patient Identification

Numerical OrderProcedure
1 Procedure Patient identification involves at least 3 steps.
2 Procedure "Sir, please a . state and b. spell your full name and c. dob" Do not ask yes/no questions.
3 Procedure Compare with info on patient's ID band or document alternative ID such as government issued photo ID.
4 Procedure For a child, nurse/parent confirmation is accepted.
5 Procedure Confirm information with lab labels and/or requisitions
6 Procedure If all 3 Identification steps match, proceed with the procedure.
7 Procedure Documentary the dietary facts, if necessary.
8 Ask Pertinent Questions Have you been fasting?
9 Ask Pertinent Questions Do you have a latex allergy?
10 Ask Pertinent Questions Do you have a preferred site?
11 Ask Pertinent Questions Have you had a fistula?
12 Ask Pertinent Questions Have you had a mastectomy?
13 Ask Pertinent Questions Have you ever fainted?
