Personal Health Records/ Patient Portals

| Advance Directive | binding legal document prepared and signed by a competent individual outlining the person's wishes should the person become incapacitated; for ex, a medical power of attorney and a living will. |
| caregiver | person responsible for providing physical care and emotional support, usually in a home-care setting, to a person who is ill, disabled, or dependent. |
| host | server that provides data transfer, storage space, and other services to users at remote locations; a host has a unique domain name and is, in effect, the point at which a website originates. |
| living will | part of an advance directive that specifies which life-sustaining treatments (for example, mechanical ventilation and tube feeding) should be administered or withheld if the person becomes incapacitated. |
| medical power of attorney | (also called durable power of attorney for healthcare or healthcare proxy) The part of an advance directive naming a trusted person to make medical decisions on the patient’s behalf should he or she become unable to make such decisions independently. |
| online community | virtual meeting space where like-minded people with common interests or concerns interact and build relationships using real-time chat rooms, etc. |
| patient-controlled health records | portion of a patient portal that contains data loaded by the patient and to which he or she alone may grant or deny access. |
| patient portal | website that serves as an information transfer hub between patient and physician and that provides information and services, such as secure email, search capabilities, access to an online appointment book, and limited access to patient records |
| personal health records (PHR) | A secure, comprehensive record of health information that is controlled by the individual, creating a confidential electronic or paper-based file that is easy to access, manage, and share. |
| populate | complete a template or create a record by filling in a set of predetermined fields with information ranging from demographic data to values and measurements (for example, vital signs) to entire documents (such as correspondence and operative reports). |
| social networking | practice of using online communities to expand one’s social or business contacts and to exchange content, such as images and instant messages; social media, etc. |
| Wi-Fi | (short for Wireless Fidelity, a technologic certification body) A means of connecting wirelessly to the Internet using a local area network or router. |
