Abbreviations used for symptoms/ exam findings

QuestionAnswer
SOB shortness of breath
NAD no acute distress (patient does not display intense symptoms)
A&O alert and oriented (patient is aware of what is going on)
CTA clear to auscultation (description of normal sounding lungs)
WNL within normal limits
WDWN well developed, well nourished (patient is growing/ has grown appropriately, does not appear malnourished)
RRR regular rate and rhythm (description of normal heart)
CV cardiovascular
HEENT head, eyes, ears, nose and throat
PERRLA pupils are equal, round, and reactive to light and accommodation
