Introduction to Health Records

TermDefinition
Subjective The information the patient says to the health care provider.
Objective The information that reveals the test results of patient.
Assessment The analysis of information performed by the health care provider through the subjective and objective information.
Plan The course of action for the patient to perform after receiving the assessment information.
Cranial Referring to the head.
Caudal Referring to the tail.
Ipsilateral The controlling hemisphere and its subject are located on the same side.
Contralateral The opposite hemisphere provides the control.
Medial Meaning closer to the center of the body.
Lateral Meaning further from the center of the body.
