form

TermDefinition
CMS-1500 has 33 blocks (items) divided into 3 sections. must have patients and guarantor's demographics and insurance info, diagnostic test, treatment or procedure info and billing info. used for paper claims.
CMS-1500 sections carrier block, patient/insured, physician/supplier
section 1 carrier block
section 2 patient/insured
section 3 physician/supplier
carrier block section contains the address of the insurance carrier and is located at the top of the form.
patient/insured section contains info about the patient or insured (if other than patient). includes boxes 1-13.
phyician/ supplier section contains info about the physician or supplier. includes boxes 14-33.
