patient care coordination and education

TermDefinition
determinants of health several social factors, health services, individual behaviors, and genetic factors.
social determinants are the accessibility to meet daily needs such as education and employment opportunities, grocery stores with adequate fresh foods, minimal exposure to crime and violence, adequate transportation, exposure to mass media and emerging technologies.
physical determinants are the influence of the natural environment on health including weather or climate change, housing and neighborhoods, work sites, and recreational settings.
MACRA replaces the former Medicare reimbursement schedule with anew pay-for-performance program that focuses on quality, value, and accountability.
payment tracks available through MACRA the merit-based incentive payment system (MIPS) and advanced alternative payment models (APM). these tracks determine what Medicare will reimburse providers.
four performance categories quality, cost/resource use/ clinical practice improvement activities, advancing care info.
pre-visit planning check due dates of preventive testing, due dates of immunizations, due dates of patient care management items, and expired or soon to be expired prescriptions
patient-centered medical home (PCMH) a model philosophy intended to improve the effectiveness of primary care
accountable care organization (ACO) an association of providers and third-party payers that assumes a defined range of responsibilities for a specific population and is held accountable, financially as well as through specific quality indicators for its members health
holistic health care comprehensive or total patient care that considers the physical, emotional, social, economic and spiritual needs of the person
primary care provider (PCP) first provider the patient seeks care from. main goal is to coordinate preventative health care services. can be family practitioners, internal medicine or doctors of osteopathy, or pediatricians.
specialist a provider that diagnoses and treats conditions that require a specific area of expertise and knowledge. PCP's may refer patients to a specialist to diagnose or treat a specific short term condition. chronic disease, long term.
physician assistants (PAs) similar training to physicians and are licensed to practice medicine as long as they are supervised by a MD. can conduct physical exams, provide preventative care, prescribe diagnostic tests, assist with procedures, diagnose illnesses, prescribe medicine
advanced practice nurses have more education and experience than RNs. can usually perform the same tasks as a physician asst. clinical nurse specialists, nurse anesthetics, NP, and nurse midwifes are common advanced practice nurses.
registered nurses (RNs) licensed by individual states and have an associate or bachelors degree in nursing. usually oversee the case management of patients who have complex chronic conditions, also coach patients about overall health.
Practical nurses (PNs) referred to as vocational nurses and also licensed by individual states. usually train for approx. 1 year at a community college or vocational school with diploma or associate degree. often triage calls, administer medicine, assist w/ clinicals
pharmacists prepare and dispense medications. must be knowledgeable of individual and various combinations of medicine.
dentists diagnose and treat issues relating to the teeth and mouth. also educate patients on ways to prevent problems with oral health.
occupational therapists assist and educate patients on how to perform everyday tasks after physical, mental, or developmental disability has occurred.
physical therapists asses a patients pain, strength, and mobility and then develop a treatment plan to improve any areas of concern.
speech therapists work with patients who have a problem with speech and swallowing due to an injury, cancer, stroke. focus on helping a person to improve, regain, and maintain the ability's to communicate, chew, swallow.
psychiatrists MDs who diagnose, prescribe medications, and treat mental, behavioral and emotional disorders.
psychologists not MDs but have a doctor of psychology or a doctor of philosophy. work with patients who are expecting mental health challenges, especially during times of stress or emotional turmoil.
social workers assist patients and families in times of transition or crisis. assist patients in a clinical or hospital setting with physical, emotional, and financial issues related to illness or injury.
support staff clinic coordinator, medical billing specialist, financial counselor, scheduler, medical records specialist, clinical medical assistant, medical administrative assistant.
three goals of institute for health care improvement (IHI) improving the experience of care, improving the health of populations, reducing costs of health care.
three goals of affordable care act (ACA) expand health insurance coverage, shift focus of health care delivery system from treatment to prevention, reduce costs and improve the efficiency of health care.
continuity of care continuation of care smoothly from one provider to another, so that the patient receives the most benefit and no interruption to care.
