NKDA No Known Drug Allergies
NAD No Acute Distress
QD vs. QID Each day ("quaque die") vs. Four times a day ("quatar in die")
RR vs. RRR Respiratory Rate vs. Regular Rate and Rhythm (of the heart)
CC Chief Complaint; the main reason for the patient's visit
prophylaxis preventive treatment
HPI History of Present Illness; the story of the patient's problem
O part of SOAP method Objective; data collected to assist in understanding the nature of the problem, including the exam, lab results and imaging studies
acute recently started or sharp, severe (referring to a symptom)
PCP Primary Care Provider
