Medical Terms and Definitions

TermDefinition
Assessment Analysis of subjective and objective information performed by the health care provider.
Objective What test reveal.
Subjective What the patient says.
Plan The course and action for the patient.
Health Record The patient's information; such as illnesses, treatments, continuing medical problems(if any), family health history and/or current medications(if any).
Medical Assistant Trained to carry out basic administrative and clinical tasks under the guidance of a health care provider.
Pediatrician A physician with special training in caring for children.
Speech Therapist Specially trained in evaluating and treating problems with speech and/or swallowing.
Patient's Social History A record of habits such as smoking, drinking, drug abuse, and/or sexual practices that can impact their health.
Family History Any significant illnesses that run in the patient's family.
