Brackets [ ] | encloses synonyms, alternative wording, or explanatory phrases. Found in Tabular List. |
Parentheses ( ) | encloses supplementary words (nonessential modifiers) that may be present or absent in the statement of a disease/procedure without affecting code assignment. |
Colon : | Used after an incomplete phrase/term that requires one or more of the modifiers indented under it to make it assignable. Located in Tabular List. |
Brace } | used to enclose a series of terms, each of which is modified by the statement appearing at the right of the brace. |
Includes | Indicates seperate terms, such as, modifying adjectives, sites and conditions, entered under a subdivision to further define or give examples of, the content of the category. |
Excludes | guideline to direct the coder to the proper code assignment. |
Notes | used to define terms and give coding intructions. |
See | indicates that another term should be referenced. |
See Category | refers coder to a specific category, must always be followed. |

Primary code | ICD-9-CM code that defines main reason for current encounter. |
manifestation | characteristic signs/symptoms of an illness. |
Etiology | cause(s) or origin of a disease. |
Eponyms | medical procedures or conditions named after a person or a place. |
cause | that which brings about any condition or produces any effect. |
acute | refers to the condition that is the primary reason for the current encounter. |
