| Term | Definition |
| Lethargic lah-THAR-jik | A decrease in level of consciousness; may be physical or mental, but generally indicates that the patient is very sick. |
| Oriented OR-ee-EN-ted | Patient is aware of who he/she is, where he/she is, and the time (Aware of person, place, and time). |
| Percussion per-KUH-shun | The processes of hitting something and listening to the resulting sound/feel to estimate the size/borders/consistency of organs; or to determine if there is fluid in an area. |
| Sequelae seh-KWEL-ah | Problem resulting from disease or injury; Ex: Poor blood flow to feet as a result of diabetes (Neuropathy). |
| Palliative PA-lee-ah-tiv | Treating symptoms, but not getting rid of the cause. The goal is to improve quality of life for family and patient (Ex: Hospice). |
| Contralateral KON-trah-LA-ter-al | Opposite side. Ex: A stroke of the right side of the brain could cause contralateral (in this case, left side) paralysis. |
| Ipsilateral IP-sih-LA-ter-al | Same side. Ex: A tumor on the right side of the brain may only affect vision ipsilaterally (in this case, right side). |
| NKDA | No Known Drug Allergies; patient does not have any documented allergies (is a part of patient's medical history). |
| CTA | Clear To Auscultation; description used to describe the patient's lungs during assessment (means no abnormal lung sounds). |
| Disposition dis-poh-ZIH-shun | What happened at the end of a visit; used to reference where patient went after a visit (the plan for continuing health care of a patient after discharge). |
