Therapy-   is the attempted remediation of a health problem, usually following a diagnosis. It’s synonymous with treatment in the medical field

HEENT =   head, eyes, ears, nose, throat

SOAP-   acronym for subjective data, objective data, assessment, plan (the way progress notes are organized)
