L in oLd carts Location - location of injury, pain, pressure, etc
D in olD carts Duration - is it intermittent, constant, sporadic, etc
C in old Carts Character - is the pain stabbing, burning, stinging, Is the cough productive, non-productive, etc
A in old cArts Aggravates - what makes the symptoms worse
R in old caRts Relieves - what gives relief or what OTC or other remedies have been tried
T in old carTs Timing - when does it occur, how often, etc
S in old cartS Severity - on a pain scale 0-10 with zero being no pain and 10 being the worst possible where does it rank
