What is the chain of Command? timed written record of different individuals who have custody of a report from initial aquisition to the final disposition in a court as evidence.
Define QA Quality assurance
What is appropriate documentation on a patients chart? Signs, symptomes, and changes in the patients condition
How do you properly correct a mistake on a patients chart? By marking it through with a single line and initialing the change.
What is subjective information? something the patient or their family tells you
What is objective information? information collected by your own senses.
What are signs? Objective information
What are symptomes? Subjective information
Where do you dispose of a used needle? in a sharps container
What hold a catheter in place? a balloon of water at the end of the tube
How do you know how much water the balloon at the end of a catheter will hold? The amount is printed on the catheter When removing a Catheter
what should you do if you meet resistance? wait a few minutes and try again.
