Subjective Anything that the patient can communicate to medical professionals about his or her condition
Objective Care professionals record what they can observe through tests, touch, and measurements regarding the patient's condition.
Assessment Care professionals use what they have found in the previous two sections to make a diagnosis of what is wrong with the patient.
Plan Care professionals plan the course of treatment based on the diagnosis.
Afebrile To not have a fever
Lethargic An indication that the patient is really sick
Chief complaint The main reason for the patients visit
PO By mouth
NKDA No known drug allergies
NPO Nothing by mouth
