What is the SOAP Method? The SOAP Method is an approach providers use to solve medical problems; also known as a SOAP note.
SUBJECTIVE How a person experiences his or her problem, as well as personal and family medical histories.
OBJECTIVE DATA; Compromise the patient's physical exam, any laboratory findings, and imaging studies performed at the visit.
ASSESSMENT Forms an analysis. Diagnosis of a problem; or a list of possibilities for the diagnosis, also know as a DIFFERENTIAL DIAGNOSIS.
PLAN Course of action with the patients assessment; or collecting more data to receive a more accurate diagnosis.
