The SOAP Method, Common Terms and Types of Health Record, Abbreviations

Medical Assistant A medical personnel trained to carry out basic administrative and clinical tasks under the guidance of a healthcare provider
Family History Any significant illnesses that run in the patient's family
Pediatric Intensive Care Unit PICU
Differential diagnosis A list of possible causes of the patient's problem or complaint
follow up f/u
Distal Farther away from the center
Reassurance To tell the patient that the problem is not serious or dangerous
Coronal Divides the body into slices from front to back
Blood Pressure BP
Prescription A type of note that doesn't follow the SOAP note format - because it is the plan.
