leave Application Process

In situations where the employee knows in advance that he will need a period of medical leave (e.g. for childbirth, adoption, recuperation from surgery etc.), he should file the request for leave as soon as he knows the dates
that he will need to be on leave (sometimes the date will be imprecise, such as with childbirth
If the employee knows that he will need FMLA leave, he should give as much notice as possible
but not less than is required under the applicable leave plan
Sometimes emergencies require leave that was not planned or Anticipated
When this occurs, the employee should file the application for leave as soon as it is reasonably possible
The Certification of Health Care Provider applies only to leave for the condition specified
If the employee later requires FMLA for a different purpose he will need to obtain a separate Certification
When an employee requests FMLA for a family member’s serious health Condition he must complete and submit Form WH–380–F, Certification of Health Care Provider
The employee will not be requested to provide such an update more often Than every thirty (30) days
unless his health care provider specifies a more frequent period or unless there is reason to believe his condition May have changed
If the condition is described as “lifetime” or “Unknown a recertification may be requested no more than every six (6) Months
If the employee must request an additional leave of absence for the same serious health condition within a 12–month period from the initial acceptance of his Health Care Provider Certification
he may be required to provide additional documentation (i.e., recertification) based on the certification he provided previously
