When Filling CG-4910, Gather Information From? Obtain 3307’s from SPO, containing supporting documentation
Where to Look up the Offenses? UCMJ
Where to List the UCMJ Article Numbers on the CG-4910? Details of Offenses
After the CO Has Determined the Punishment, Retain Original CG-4910? In the unit punishment log for 4 years
A CG Memo is Created? To document the information from the mast to execute the punishment at the unit
CG Memo Copies? 1 original and 2 photocopies, original is filed in the MBRs SPO PDR, Sec 2, give a copy to the MBR and send a copy to the MBR sup or place of restriction if applicable
Mast Package in the Unit Punishment Log Should Include (5)? Original CG-4910, Acknowledgement of Rights, PIO Report, Original Witness Statements and CG-3307 terminating/restarting GC eligibility
Following NJP, Enlisted? Complete an EER with type as NJP and review type as DISCIPLINE
Following NJP, Officer? Complete a special OER
Following NJP, Non-Rate? Cancel “A” School if applicable
Following NJP, Reduction to E-3 or Below? If assigned to sea duty and previously entitled to BAH, no longer authorized effective the reduction date
Reduction BAH Restrictions W/Dep or Paying Child Support? BAH Partial or BAH Diff, not entitled if spouse is AD
DA Transactions? Must complete all tabs on the Disciplinary Action page
After Printing the Disciplinary Action Page? File it in Sec 2 of the MBRs SPO PDR and send a copy to PSC (PSD-MR)
