Amendment of birth certificate Filing a change with the state because information on the original birth certificate was incorrect or omitted
Dictation system The recorders on which physicians dictate reports and the management software that lists each dictation job by date, time, dictator, report type, and so forth
Paternity declaration A legal document used to establish paternity of a child when the mother and father are not married to each other
registration of birth completing the required birth certificates form and filing it with the state department of health
transcription log A list of all dictation recorded by date received and transcribed , type of report, patient name, and physician dictating
transcription system the computers on which transcriptionists produce reports and the word processing software that assists them
