| No Helper | Level 5-7 |
| Helper Modified Dependent | Level 3-5 |
| Helper Complete Dependent | Level 1-2 |
| Code 0 | Code 0 indicates that the patient did not perform the activity, and that a helper did not perform the activity for the patient, during the entire assessment time frame. Possible reasons for a patient’s nonperformance of an activity include: |
| Level 7 | Complete Independence (timely, safely) |
| Level 6 | Modified Independence (device, safety, extra time) |
| Level 5 | Supervision/Setup (cueing, providing setup, coaxing) |
| Level 4 | Minimal Assistance (patient performs 75% or more of activity) |
| Level 3 | Moderate Assistance (patient performs 50% to 74% of activity) |
| Level 2 | Maximal Assistance (patient performs 25% to 49% of activity) |
| Level 1 | Total Assistance (patient performs less than 25% of activity) |
