Correction of Errors You must in ink draw one single line through the error in the chart and rewrite the correction next to the error and initial it.
Paperless New software systems available to dental offices in which there is no hard file of dental charts.
Health History Must be updated at every visit.
Legal Documents This is what patient records are, and must be kept in a physically secured area.
Notice of Privacy Practices To maintain accordance with HIPPA, every patient is to sign an acknowledgement that they received it.
Minimum Information Name, date of birth, address, emergency contact phone #, employment and attitude toward dentistry, including expectation of care.
7 Years State law mandates that dental records be kept for at least this long.
Chronological Forms in the patients record are placed with the most recent information on top.
Blue or black ink The acceptable writing instrument for documenting in a patients dental record.
