Descriptions and Symbols

Outlined and filled in with red when it needs to be done, blue when completed. Amalgam Restoration
Outlined in red when it needs to be done, blue when completed. Some offices use a outlined red or blue circle with small dots. Composite Restoration
Outlined and filled with diagonal lines - red when needs to be done and blue when complete. Gold Crown Restoration - FGC
Outlined with a P inside in red or blue depending on status. Outlined in red or blue only in most offices. Porcelain Restoration
Outlined with Criss Cross pattern or with SS or SSC inside the outline, red when needs to be done and blue when it is complete. Stainless Steel Crown
Large X through multiple teeth in blue. Multiple missing teeth
Blue circle around entire tooth and root. IMP or UE Teeth that are impacted or unerupted
An arrow in the direction which the tooth is drifting or inclination near the tooth. Teeth that are drifting/or inclined (ex. mesial drift)
An arrow up or down depending on arch Tooth that is drifted in an overeruption/super eruption pattern.
Diagonal red line or 2 lines through a tooth or teeth Teeth to be extracted
Circle and large X in blue. Most will use a large X through entire arch. All missing teeth
Straight line through root and triangle at the end (apex0 in blue or red. Tooth with root canal treatment, apicoectomy, and silver amalgam retrofill.
Straight red line through root only. Teeth that need root canal therapy (treatment)
Straight blue line through root only. Teeth with completed root canal
Red circle at the apex or tip or root. Tooth with an abcess
Outlined crown of tooth on facial/buccal with occlusal and/or lingual is outlined and diagonal lines going through only this area. Tooth with a porcelain fused to metal crown
Pontic will be marked with a X where tooth is missing. Missing tooth area in a bridge and name of the fake tooth on bridge is called Pontic
Abutment has teeth still present but are the anchor teeth for a bridge Fixed bridge will have teeth that anchor the Pontic tooth in place with a bridge. Abutment teeth are the teeth still present but have crown/bridge preps on them.
Extra tooth symbol noted where extra tooth is located in blue Supernumerary teeth
Outlined facial of tooth in blue or red and the lingual portion is outlined in blue or red with diagonal lines with a hook sign on two adjacent teeth on both sides Maryland bridge
Circle in Red or Blue on surface with the Initials under the root of material type ex. IRM Tooth with temporary restoration
Triangle in blue or red in the interproximal area of filling material that extends in the IP area. Overhang on a restoration
