Class III mobility Charted in red with roman murmeral III on the tooth in question.
Root canal with porclain fused to gold Crown Charted in blue with a line down the root and ///// lines through the crown of the tooth in question.
Occlusal amalgam Charted in blue on the occlusal surface of the tooth in question.
Decay on 3 MOL Charted in red on the mesial and occlusal of #3.
Heavy calculus Charted with CCCCCCCCC on the teeth in question.
