Licensing Requirements

temporary license issued for opening a new center
temporary license issued for relocating a center
temporary license issued for changing ownership of a center
temporary license issued for up to 6 months
provisional license issued if fail to meet minimum standards
provisional license issued for up to 12 months
special provisional license issued when child abuse or neglect has occurred in a center
special provisional license issued for up to 6 months
probationary license issued when violations willful, continual, or hazardous to safety of children
probationary license issued for up to a year
rated license issued based on program standards, education level of staff, and compliance history
rated license issued at end of 6 month temporary license
changes in ownership cannot be bought, sold, inherited, or transferred
changes in location must notify the division 30 days prior to change
changes in capacity can be requested in 2 situations - space previously owned not used or new space
