Anti Anginals

TermDefinition
Nitrates Amyl nitrate, Nitroglycerin, Isosorbide dinitrate
Amyl nitrate inhaled, 3-5min duration of action
Nitroglycerin Absorbed by GI but metabolized by liver. Use aerosol or sublingual. Transdermal (8-10hour duration)
Isosorbide dinitrate Oral, sublingual. 4-6hour duration (due to active metabolites). Keeps NO from dissipating.
Nitrates Side FX Major effect: pooling of blood headache, hypotension, reflex tachycardia.
Calcium Channel Blockers Dihydropyridine, Benzothiazepine, Phenylalkylamine NOTE: Dilitiazem and verapamil have active metbaolites.
Dihydropyridine Ex: nifedipine. Potent vasodilator, reduce BP quickly, may increase heart rate, no effect on AV conduction
Benzothiazepine Ex: dilitiazem mild slowing of AV conduction, moderate peripheral/coronary vasodilation, mild decrease in heart rate.
Phenylalkylamine Ex: Verapamil, slows AV, moderate vasodilator, dilation of coronaries, peripheral vasodilation, good absorption. Hepatic metabolism.
Calcium Channel Blockers SIDE FX Side effects: Peripheral edema (N > V > D), flushing dizziness (N), constipation (if too long use) (V) Potential toxicity (av block, hypotension, heart failure.
Calcium Channel Blockers USES Uses: Prevent focal coronary artery spasm, management of angina, superventricular tachycardia, improvement of exercise endurance.
Atenolol B1 blocker
Metoprolol B1 blocker
Propanolol Non selective B Blocker (not good for angina in asthmatics)
Beta Blockers Atenolol, Metaprolol, propanolol
