DMARDS and Steroids

Infiximab Binds TNFa. Trade name Remicade.
Adalimumab Binds TNFa
Monocolonial Antibodies Adalimumab, Infiximab
Recombinant Protein Lenercept, Etanercept
Lenercept Binds TNFa
Etanercept Binds TNFa. Trade name Enbrel.
Other DMARDS Cyclosporin, Hydroxychloroquine, Colchicine, Methotrexate, Azathioprine
Cyclosporin Inhibits transcription of interleukin 2 (IL-2) gene.
Hydroxychloroquine Antimalarial but used for SLE and rheumatoid arthritis. Increase cell pH, interfere with proteolysis and antigen presentation. Inhibits Ca2+ signaling necessary for T cell activation.
Colchicine from crocus flower, prevents migration of neutrophils (via microtubule depolarization). Used for gouty arthritis.
Methotrexate Inhibits dihydrofolate reductase and thymidylate synthetase. Limits DNA and RNA synthesis.
Azathioprine Metabolized to mercaptopurine. Inhibits clonal expansion of B and T cells. Inhibits DNA synthesis. Used as an immunosupressent. Can cause “Stevens-Johnson” syndrome and epidermal necrolysis.
Corticosteroids and Anti-inflammatories Lipcortin, Fludrocortisone, Betamethasone, Prednisone, Beclomethasone, Prednisolone, Mometasone
Lipcortin Also called Annexin I, inhibits PLA2, prevents phosphatidyl choline → arachidonic acid. Glucocorticoids increase its transcription.
Fludrocortisone Mineral corticoid (mild anti-inflamatory properties).
Betamethasone Glucocorticoid (mild mineralcorticoid properties). 1-3 day half life. For Dermatological problems ex: contact and seborrheic dermatitis. Pruritis.
Prednisone for MS, Ulcerative colitis, and Lupus erythematosus. MILD glucocorticoid
Prednisolone for MS, UC, and LE. MILD glucocorticoid
Beclomethasone Respiratory problems, respiratory distress infant syndrome. Asthma, given via nasal inhalation or oral inhalation.
Mometasone Respiratory problems, respiratory distress infant syndrome. Asthma, given via nasal inhalation or oral inhalation.
Corticosteroids and Anti-inflammatories Side FX Hyperglycemia, Osteoporosis, cataracts, glaucoma, body fat redistribution, Cushing’s Disease.
