Transferring a Resident from a Wheelchair to a Bed

| Step 5 * | Adjust bed to lowest position, lock bed wheels, and raise head of bed to Fowler’s or sitting position.

| Step 6 | Fanfold top linens to the foot of bed.

| Step 7 | Observe resident is wearing non-skid footwear. Stand in front of the resident. Apply transfer belt. (must be tight – a flat hand should slide snug between belt and body)

| Step 8 | Ask the resident to hold onto the armrests and lean forward. Grasp the transfer belt at each side.

| Step 9 | Position your feet and legs to provide stability for the resident and prevent the resident from falling or sliding.

| Step 10 | Ask the resident to push down on the armrests on the count of three. Pull resident into a standing position as you straighten your knees.

| Step 11 | Support the resident in the standing position.

| Step 12 | Turn the resident so they can reach the edge of the mattress. The back of the resident’s legs should touch the mattress.

| Step 13 | Continue to turn the resident until they can reach the mattress with both hands.

|
||||
| Step 14 | Lower the resident onto the bed as you bend your hips and knees. The resident assists by leaning forward and bending the elbows and knees.

| Step 15 | Remove the transfer belt and footwear.

| Step 16 | Assist the resident to lie down. Provide for comfort.

| Step 17 * | Attach signal light within the resident’s reach.

| Step 18 | Perform hand hygiene and report and record observations.
