What is the classic presentation of polycythemia vera? Average age of onset 50-60 yrs, visual distubances, thrombosis(15%), Erythromelegia(burning pain in the hands and feet with erythema, pallor, or cyanosis), pruritis, facial plethora, hepatosplenomegaly.
What lab findings are seen in polycythemia vera? elevated H&H and red cell mass, basophilia, leukocytosis (40%), thrombocytosis(60%)
What is tx for polycythemia vera? phlebotomy to keep Hct <45% in men and 42% in women, add hydroxyurea (if at high-risk for thrombosis), ASA qd prevent thrombosis, IFN-a(if refractory pruritis or erythrocytosis), allopurinol(if hyperuricemia)
A 66 y/o woman fractures her hip after falling from standing. XRAYS show "punched out" lesions in vertebrae, hips & femurs. She has increasing back pain, weakness, fatigue. Labs show anemia, hypercalcemia, & increase BUN & Cr. What tests should you do? She has MM. Get SPEP and UPEP and bone marrow biopsy(to make a definitive diagnosis).
A 21 y/o male pt. presents with recent weight loss, pruritis, and night sweats. PE reveals hepatosplenomegaly and a nontender cervical lymphadenopathy. What do you immediately suspect? Hodgkins lymphoma
