Cardiac Focused Assessment

| Introduce yourself and explain what you are going to be doing to the patient. | |
| Wash Hands, provide privacy, and raise the bed to a comfortable working level | |
| Start by inspecting the patient for any abnormalities like Edema or Cirrohis in the feet and lower extremities. | |
| Palpate dorsalis pedis, posterior tibialis, popliteal, & femoral pulses and check for symmetry. | |
| Inspect the patients upper extremities for any abnormalities and palpate and check for symmetry of the radial pulses. | |
| Assess Capillary refill is <3 seconds. | |
| Inspect & palpate PMI. (point of maximal impulse) @ apex of the heart, approximately mid clavicular 5th intercostal space. | |
| Auscultate PMI. | |
| Inspect, Palpate, & auscultate (with bell & diaphram) the aortic, pulmonic, tricuspid, mitral heart sounds of S1 & S2 or extra heart sounds S3 & S4. Making note of any abnormalities such as: Bruits (murmurs) (All Patients Take Meds) | |
| Check the carotid arteries and jugluar veins, inspect for JVD | |
| Once complete, make sure the patient is comfortable, lower the bed, raise the bed rails, put the call bell in reach and wash your hands. | |
| Document the procedure. |
