ct has DVTand has been taking heparin for 1 wk. 2 days ago PCP prescribed coumadin. ct questions nurse about being on heparin and coumadin at the same time. which response by the nurse is appropriate? warfarin takes 3-4 days to achieve therapeutic anticoagulant effects. heparin will be discontinued soon.
