SAMPLE. S= Signs and Symptoms. A= Allergies. M= Medications. P= Pertinent past medical history. L= Last oral intake. E= Events leading up to the incident.
OPQRST. O= Onset. P= Provocation. Q= Quality. R=Region/Radiation. S= Severity. T= Time.
APGAR. A-Appearance. P-Pulse. G-Grimace. A-Activity.
SPLATT S-Symptoms. P-Previous falls. L-Location. A-Activity at fall. T-Time of fall. T-Trauma.
