AZ Drug Dosages

Dosages for AZ paramedic medications

QuestionAnswer
Adenosine 6 mg rapid IVP followed by 20 mL saline flush. May repeat with 12 mg rapid IVPx2 in 1-2 minutes.
Albuterol 2.5 mg of premixed solution via inline SVN
Amiodarone - VF/Pulseless VT 300 mg IVP may repeat with 150 mg IVP
Amiodarone - Post Resuscitation/Conversion 1 mg/min for first 6 hours then 0.5 mg/min for 18 hours. Max daily dose of 2.2 g
Amiodarone - Wide-Complex Tachycardias, A Flutter, A Fib, SVT with cardioversion 150 mg over 10 minutes (mix in 50 mL bag of D5W)
Aspirin 162-324 mg (2-4 chewable 81 mg)
Atropine - Symptomatic Bradycardia 0.5 mg. May repeat in 5 minutes with 1.0 mg max of 3.0 mg
Atropine - Organophosphate poisoning 1-5 mg. Double dose every 5 minutes until SLUDGE stops. Infusion 0.5-1 mg/hour or 10%-20% of loading dose/hour
Calcium Chloride - Hypocalcemia, calcium channel blocker OD, hyperkalemia and hypermag 5-10 mL (0.5-1g) of 10% calcium chloride. May repeat in 10-20 minutes
Dextrose 50%, D50 Hypoglycemia, ALOC - 25-100 mL (12.5-50g, 1/2 to 2 amps) Hyperkalemia - 50g over 1 hour
Diazepam Valium 2 mg slow IVP; No faster than 5 mg/min Status Epilepticus: 5-10 mg IVP. May repeat in 10-15; Max of 30mg
Diphenhydramine HCl Benadryl Allergic Reaction/Anaphylaxis: 25-50 mg Slow IVP Extrapyramidal symptoms/antiemetic: 10-50 mg
Diltiazem Cardizem 0.25 mg/kg IVP over 2 minutes. Repeat in 15 min with 0.35 mg/kg IVP if inadequate response Infusion: 5.0 to 15 mg/hr
Dopamine Prep: 400mg/250 mL NS or Dextrose =1600 mic/mL Brady: Start at 5 mic/kg/min Shock: BP <70: Start at 5 mic/kg/min. BP >70: Start at 2.5 mic/kg/min
Epinephrine - Pulseless Arrest 1mg of 1:10,000 ever 3-5 minutes
Epinephrine - Continuous Infusion for Hypotension or Symptomatic Bradycardia 1mg added to 500mL of NS administered at 2-10 mcg/min
Epinephrine - Anaphylaxis and Asthma 0.3 to 0.5 of 1:1000 IM, SC, SL; May repeat every 15-20 min. Extreme cases may use 1:10,000 and give 0.1 mg every 5 min IV/IO
Furosemide Lasix. 0.5 to 1.0 mg/kg IVP; Max dose of 40 mg. CHF/Pulmonary Edema: 20-40 mg IV over 1-2 min
Glucagon 1 mg IM; May repeat in 20 minutes
Ipratropium Bromide Atrovent. 500 mcg in 2.5 mL NS (1 unit dose vial) via inline SVN. May mix 1 unit dose vial of atrovent with 1 unit ddose vial of albuterol
Lidocaine HCl - Pulseless VF/VT Initial bolus of 1.0-1.5 mg/kg IVP 3-5 min to a total of 3 mg/kg. Bolus of 1.5 mg/kg for cardiac arrest
Lidocaine HCl - Antidysrhythmic Initial bolus of 1.0-1.5 mg/kg IVP; 0.5-0.75 mg/kg every 5-10 min total of 3 mg/kg.
Lidocaine HCl - Maintenance Infusion Add 1-2g to a 250 mL NS or D5 and start at drip rate of 2-4 mg/min.
Lorazepam Status Epilepticus: 2-4 mg slow IV (over 2-5 min). May repeat in 10-15 min to max of 8 mg
Magnesium Sulfate 1-2 g IV diluted in 50-100 mL NS of D5 over 1-2 minutes followed by same amount over 1 hour
