Adult / Pedi Trauma alert

| Adult Airway HARD? | Active airway assistance beyond the Administration of O2. |
| ADULT HR HARD? | HR > 120 without raidial pulse |
| ADULT BP HARD? | SBP < 90 |
| ADULT GCS HARD? | Best motor response =<4 or GCS =<12 |
| ADULT BURN HARD? | 2nd or 3rd degree burns on 15% or > TBSA |
| ADULT Amputaion HARD? | Proximal to wrist or ankle? |
| ADULT Penetrating injuries HARD? | Penetrating injuies to Head, Neck, or Torso excluding superfical wounds where depth can be determined. |
| ADULT Fx HARD? | 2 or more Long Bone Fx site. (humerus, radius, ulna, femur, tibia, fibula) |
| ADULT PMS HARD? | Paralysis, loss of sensation, or suspected Spinal Cord Injury |
| ADULT Airway Soft? | RR => 30 |
| ADULT HR soft? | Sustained HR =>120 Bpm |
| ADULT GCS soft? | GCS best motor response = 5 |
| ADULT Avulsion soft? | Major Degloving injury or flap avulsion > 5 inches |
| ADULT GSW soft? | GSW to any extremity |
| ADULT Fx soft? | One long-bone Fx from MVC or fall => 10 ft |
| ADULT Age soft? | =>55 yrs |
| ADULT MVC soft? | Ejection / thrown from any vehicle (including ATV, Motorcycle, Moped, or Truck bed) |
| ADULT interior MVC soft? | Steering wheel deformity. |
| Always Have for Trauma Alert Criteria? | PARAMEDIC judgement with proper documentation in Report. |
| PEDI Airwary HARD? | Active airway assistance beyond administration of O2. Any Airway Adjunct including Manual Jaw thrust, Suctioning, or other Ventialation assistance. |
| PEDI LOC HARD? | Altered Mental Status |
| PEDI PMS HARD? | Paralysis, loss of sensation, or suspected Spinal Cord Injury |
| PEDI Pulse HARD? | Faint or nonpalpable Carotid or Femoral Pulses. |
| PEDI BP HARD? | SBP < 50 |
| PEDI Fx HARD? | Open long bone Fx, multiple Fx, or Dislocation sites. |
| PEDI Avulstion HARD? | Major Degloving or Flap Avulstions |
| PEDI Burns HARD? | 2nd or 3rd degree burns on => 10% TBSA |
| PEDI Amputaion HARD? | Amputaion proximal to wrist or ankle |
| PEDI Penetrating HARD? | Penetrating injuies to Head, Neck, or Torso excluding superfical wounds where depth can be determined. |
| PEDI LOC soft? | Suspected amnesia or Loss of Consciousness |
| PEDI BP soft? | SPB < 90 |
| PEDI Pulse soft? | Palpable carotid of femoral pulse but no radial or pedal pulse |
| PEDI Fx soft? | Suspected closed Long Bone Fx |
| PEDI Size soft? | Pt weighs =< 11 kg or body length is =< 33 inches |
